
What to Bring

Patient’s health insurance card

Patient’s photo ID
-
A list of the patient’s current providers and their numbers

A list of the patient’s current medications, including over-the-counter medications
-
Comfortable clothing including underwear, socks and pajamas (without drawstrings)- 3 outfits per patient

Dentures, glasses and other assistive devices that are clearly marked with patient name

C-Pap if you use one (we are unable to provide)

Your admission team may ask you to bring home medications or equipment in order to provide continuity of care. Please ask before bringing in any medication or equipment.
The following items will be provided to patients:

Hygiene items: soap, shampoo, toothbrush, etc.

Blankets, pillows, towels
-
Hospital gown/pants, if requested

Washer and dryer for laundering personal clothing

Writing materials and therapy items as deemed safe by our Clinical Therapist
-
Canes, walkers, and wheelchairs as deemed necessary by PT/OT
Belongings that are delivered at any time following admission will be given to the patient based on need, safety, and patient acuity. Extra items will be secured until discharge.
